Step by Step Guide
Part 1: Configure Smart Form settings
Navigate to the Smart Form page
Select Standard form type
Enter a title to name your form
Add report email address(es) where form submissions will be sent (e.g., store manager, admin)
Set up thank you message or URL (optional) for after member’s submission
Write a clear form description explaining the purpose
Configure data storage preferences:
Tick "Email only" if you do not need data stored in Eber
Leave unticked to store data in Eber system
Set points earning (optional for points to be awarded for submission)
Part 2: Create Questionnaire
Scroll down to locate the Create button to access form builder
Give your question a title under Name
Describe the purpose of asking the question
Add form fields using various field types:
Text Field: For short answers, names, or comments
Example: "Please provide your feedback about our service"
Number Field: For numerical inputs
Example: "How many people will attend the event?"
Date Field: For calendar selections
Example: "Please select your preferred appointment date"
Dropdown List: For predetermined options
Example: "Select your preferred product category:
Electronics
Clothing
Food & Beverage
Home & Living"
Multiple Choice: For selecting one or multiple options
Example: "Which of our services have you used? (Select all that apply) □ Delivery □ Installation □ Maintenance □ Customer Support"
File Upload: For document or image submissions
Example: "Please upload your receipt for verification"
